Try adding the game to the exclusions list in your antivirus software. Each time we release a new update, some AVs give us trouble. If the problems persist, we'll take a look at it.

If you ask them once you don't get the option again in that playthrough, just like the marine/life on the rails question. If it's not that, did you do anything else to him, perhaps?

They stick around for two hours of in-game time. With reloads it can perhaps seem longer.

Yes, it's unequipped so that you could pickpocket it if you have no heart. I've done some tweaking, so hopefully he'll start equipping it when needed.
